The need

We have been serving the community of Swindon and North East Wiltshire for over 25 years. During that time our unique approach to caring has extended to hundreds of patients and their families.  From this privileged position we have gained a real insight into what matters most to people, and we have endeavoured to respond with an individualised and tailor made service.

Our current specialist nursing, day hospice and in-patient services are the main focus of our care and support. They are highly-valued and much-needed, and each day make a significant and positive difference to the experience of dying people in our community.

What we know, however, is that it is not enough. Too often people are admitted to the hospice or the hospital when their real preference would be to stay in their home. Too often family and friends have become exhausted, distressed or overwhelmed by the heavy demands of caring.

It is not uncommon for people to feel desperately disappointed or even guilty that they were unable to fulfil the wishes of the person most important to them.

It is for this reason that we are embarking on a major new hospice service development.

The service

 Prospect@Home will:

  • provide additional specialist care, practical nursing and emotional support for patients, irrespective of their diagnosis
  • provide the patient’s carer’s with respite, practical help and emotional support
  • improve the overall quality of the patient’s and their carer’s lives
  • provide expert, flexible, responsive and reliable care up to 24 hours a day
  • be available wherever it is needed – in people’s homes, and in nursing and residential homes
  • ensure close liaison and working continues with the primary healthcare teams and other services involved in the patient’s care
  • provide the service free-of-charge to patients
